This book reflects years of surgical experience and aims to impart knowledge to younger surgeons and colleagues involved in patient care. A successful penetrating keratoplasty relies on several prerequisites: suitable donor tissue, careful harvesting of the donor eye and cornea, the possibility of short-term conservation, and an atraumatic transplant technique using modern corneoscleral trephination instruments, alongside a problem-oriented perioperative pharmacotherapy. These factors are crucial for the immediate success of the operation. However, the long-term success of the transplant hinges on the early recognition of both immunogenic and non-immunogenic complications starting from the patient's discharge. Given the complex interactions between the recipient eye and donor cornea, understanding these complications is vital for patient outcomes. We hope this book reaches a wide audience, as it contains essential insights for the long-term visual rehabilitation of keratoplasty patients. Special thanks are extended from the Department of Ophthalmology at the University of Cologne to the author for her dedication in directing the keratoplasty consultation service, which has greatly benefited our patients.
